Kelly 92f88fdcd6 fix(workers): Increase max concurrent tasks to 15 and add K8s permission rule 
- Change MAX_CONCURRENT_TASKS default from 3 to 15
- Add CLAUDE.md rule requiring explicit permission before kubectl commands
